Improving of Pregnancy Rate with Microsurgical Epididymal Sperm Aspiration(MESA) and Alloplastic Spermatocele Sperm Aspiration (ASSA)

Abstract
This study was carried out to analyze the results of microsurgical epididymal sperm aspiration (MESA) and alloplastic spermatocele sperm aspiration (ASSA) with intracytoplasmic sperm injection (ICSI). Sperm aspiration was performed in patients
with
congenital absence of the vas deferens (CAVD) and unreconstructable obstructive azoospermia. During the MESA operation, alloplastic spermatocele was implanted to epididymis for next aspiration. When pregnancy was failed in the first IVF cycle
used
MESA-ICSI, sperm aspiration was performed form alloplastic spermatocele at outpatient department and tried ICSI. Sperm aspirations for MESA and ASSA were 65 cases and 12 cases, respectively. In sperm parameter, sperm count was 18.3¡¿10E6/ml,
motility
was 23.8% after MESA and sperm count was 17.8¡¿10E6/ml, motility was 10.8% after ASSA, respectively. And fertilization rates of MESA-ICSI and ASSA-ICSI were 67.1% and 66.3%, pregnancy rates were 29.2% and 50%, respectively. The fertilization and
pregnancy of MESA-ICSI was successful in patients with congenital absence of the vas deferens (CAVD) and unreconstructable obstructive azoospermia. And ASSA-ICSI is able to overcome the limitation of MESA. We concluded that MESA-ICSI is a good
method
for improving fertilization and pregnancy rates and ASSA-ICSI is an effective method after the failure of pregnancy in previous MESA-ICSI.
